KAIZEN JUDO KAI, LLC and KAIZEN YOGA TEMPLE students are subject to the following Terms & Conditions.
By visiting www.kaizenyogatemple.com, a corresponding purchasing website and/or purchasing or reserving a class either
online or in an KAIZEN YOGA TEMPLE studio, you confirm you have read these terms and conditions and agree to be bound by such terms and conditions.
Reservations and Classes
To make a reservation or purchase a membership or class, clients can purchase that at the studio and / or access our online store at www.kaizenyogatemple.com.
Once a membership, single class or class package is purchased successfully, clients may reserve a class using our online services.
Drop-Ins are encouraged, however, we cannot guarantee space will be available unless a prior reservation has been made.
Clients will be immediately charged via credit/debit card when they purchase a class or series of classes online.
Clients agree to pay these charges. Clients are liable for any fees that KAIZEN JUDO KAI LLC & KAIZEN YOGA TEMPLE may incur in collecting
unpaid balances, including attorney’s fees. Unlimited membership requires 7 days notice to cancel
revolving monthly payments and clients will be charged for this time. There are no fees to cancel memberships.
Prices of future classes are subject to change.
There are no refunds for unused or partially used class packages.

KAIZEN JUDO KAI, LLC and KAIZEN YOGA TEMPLE will award any student one free class after they have referred 3 new students.
These guests must be new students to KAIZEN JUDO KAI, LLC and KAIZEN YOGA TEMPLE and pay full price for a class.
Class transfers and refunds
We cannot accommodate transferring of classes between students.
Classes are not redeemable for cash and unauthorized reproduction, resale, or trading of classes
is strictly prohibited. Refunds will not be made. Classes are refundable only if the event is cancelled and not rescheduled or upon request if the event is rescheduled.
All students are required to carefully review and agree to the to the following before attending class.
Failure to do so is the responsibility of the student.
By signing this agreement, student acknowledges that he/she is in good physical health and does not suffer
from any medical condition that would limit participation or create risk for himself/herself,
instructors or other students of KAIZEN JUDO KAI, LLC and KAIZEN YOGA TEMPLE.
Student acknowledges that KAIZEN JUDO KAI, LLC and KAIZEN YOGA TEMPLE classes can be strenuous, difficult to complete,
and can cause or aggravate minor, major or catastrophic injuries including paralysis and death.
All students should discuss their health with a medical professional prior to
beginning any exercise or fitness program, class or workshop.
If KAIZEN JUDO KAI, LLC and KAIZEN YOGA TEMPLE staff determines that participation in any KAIZEN JUDO KAI, LLC and KAIZEN YOGA TEMPLE
class would be a risk to the student, instructors, or other students, KAIZEN JUDO KAI, LLC and KAIZEN YOGA TEMPLE
retains the right to deny entry to said student until student can provide a medical opinion letter
addressing noted concerns and authorizing full participation; this is at the
sole cost of the student. All students signing this agreement
acknowledge their understanding of the risks associated with all classes, workshops or activities offered by KAIZEN JUDO KAI, LLC and KAIZEN YOGA TEMPLE.
By signing this agreement, students waive and release KAIZEN JUDO KAI, LLC and KAIZEN YOGA TEMPLE, its owners,
members, officers, employees, contractors and instructors from any claim, demand, cause of action
of any kind resulting from or related to participation in the classes,
workshops and activities offered by KAIZEN JUDO KAI, LLC and KAIZEN YOGA TEMPLE.
Student acknowledges that he/she is fully responsible for any risks, injuries and damages which might occur or be
aggravated as a result of participation in classes, workshops and activities offered by KAIZEN JUDO KAI, LLC and KAIZEN YOGA TEMPLE.
I acknowledge that I have read, understand and agree to this waiver of liability. I am legally competent to sign and agree to do so voluntarily.
KAIZEN JUDO KAI, LLC and KAIZEN YOGA TEMPLE IS IN NO WAY RESPONSIBLE FOR THE SAFEKEEPING OF PERSONAL BELONGINGS WHILE CLIENTS ATTEND CLASS.
